The BANES area has a total population of approximately 147,000 people served by Teams and Consultants in General Adult Psychiatry, Recovery & Outreach, Liaison Psychiatry, Alcohol & Addictions and Old Age Psychiatry. Bath CMHT serves seven GP practices with an approximate population of 26,000 people aged 16-65. Morbidity is similar to the national average, but with wide variation within the sector ensuring a broad clinical experience spanning `urban’ and `rural’ psychiatry. The Community Team comprises the Team and Deputy Team Leaders, 3 CPNs, 2 Social Workers, Psychologist (part-time) and Occupational Therapist (part-time). Medical input is provided by 2 Consultants, Staff Grade Doctor (1WTE) and FY2 Doctor. The Team is supported by the Hillview Lodge In-Patient Unit on the same hospital site at the Royal United Hospital, Bath. On average 9 – 12 beds are used by the Team. The Crisis & Home Treatment Team provide an alternative to admission and they work from Hillview Lodge also. The role of the Specialist Trainee will be 1) to become a member of the Community Team but also gain experience of liaising with different Teams across the service and 2) To assess and manage patients in Inpatient and Community settings, developing aspects of the service through supervision. Specialist Trainees are encouraged to take up opportunities for formal teaching at the Undergraduate Bath Academy and to gain experience in the supervision of Foundation and Core Trainees. This post is based in the City of Bath and is split between the mental health Recovery Service, (serving part of Bath City) and the early Intervention service, covering the whole of Bath & Northeast Somerset (BaNES). BaNES (population approx. 180,000) comprises the city of Bath, with two universities and a thriving tourist & other industries and the rural surrounds of Northeast Somerset. Due to a wide demographic mix including areas of relative affluence and significant socio-economic depravation, it, provides a wide variety of clinical experience, most notably within the population presenting with first onset psychotic symptoms. Local mental health services are fully integrated with local Authority services and links with commissioners and partner agencies (including a range of housing & supported living providers, voluntary agencies, addictions services, further education colleges & universities & primary care) are close and well developed. In-patient services are based at Hillview Lodge (Consultant, Dr P. Naveen), the Royal United Hospital, Bath and is supported by the Intensive Service (Consultants Dr Chris Lucchese), also working with the recovery Service) supporting the provision of alternatives to admission where possible and facilitating admission & discharge where appropriate. Our base on the RUH site has facilitated the development of a comprehensive Psychiatric Liaison Service (Consultant Dr Bill Bruce-Jones, also with the Primary care Liaison Service) We have a well-established team of Consultant Psychiatric staff, with full involvement of senior clinicians in directing locality strategy & operations, with ample opportunities for the post holder to gain experience in these areas if desired. Psychological therapies are well integrated into all areas of the local mental health service via the Psychological Therapies service We have a regular academic programme including a weekly journal club for Core & Specialist Trainees and a weekly academic programme including a range of guest speakers and case presentations, alternating between the RUH, Bath and Green Lane Hospital, Devizes. In addition to opportunies in teaching & training within the Core Trainee programme (including GP and foundation trainees), the post holder may participate in teaching 4th Year medical Students on Psychiatry placement from the University of Bristol. The Early Intervention in Psychosis Team, established in 2007 provides the opportunity to gain experience in a rapidly developing and exciting area of Psychiatry, working closely with our clients, their carers, employers & educators with highly favourable clinical & social outcomes, with 70% of clients being discharged after a period of three years to primary care. We accept referrals for clients from age 14 with first onset psychotic illness, with clients up to the age of 18 being managed by our partner CAMHS service, with a team member working part time with CAMHS. The team has representation from a range of professionals including nursing, OT, Social Workers, Psychology and a vocational specialist. The post holder will have the opportunity to gain experience in a wide range of psychosocial interventions including CBT, family work, Art Therapy, carer’s courses and supervised ASD diagnostic assessments, supervised by the Bath Autistic Spectrum Service (BASS). The Mental Health Recovery Service is a rapidly evolving service providing time-limited and longer term interventions to clients across BaNES, referred by the Primary Care Liaison Service. The post holder will work with Dr Stanton in seeing clients of working age with a wide range of mental health problems requiring a secondary service in out-patient & other settings, including “out of area” placements for clients with severe mental health problems and high levels of complex needs. Dr Stanton has been a Consultant Psychiatrist in BaNES since 1998. He has previously led local Rehabilitation and Assertive Outreach Services; although the functions of these now-disbanded services have been integrated into the Recovery service, he retains a special interest in the treatment of psychotic illnesses at all stages and the post holder will opportunity to gain experience in managing care for this client group including working with specialist residential providers and regular meetings with commissioners. |

This is an exiting new post in which the trainee will have the opportunity to experience and input to the Early Intervention (EI) for Psychosis Team in Bristol. The EI team is a general adult community team for young service users between the age of 16 and 35 years presenting with at risk state or first episode psychosis. The trainee will be an integrated member of a multidisciplinary team which offers psychiatric assessment and treatment, care coordination, CBT, family interventions, art therapy and psycho-education. The team is based at Colston Fort in the vibrant area of Cotham and in walking distance of University facilities. The objectives of the EI team are to raise community awareness, improve access and engagement to services, to promote recovery and ordinary lives, to engage families and offer support and to provide practitioners training. The aims of EI are the early detection of psychosis, the reduction of duration of untreated psychosis and to intervene early with evidence based treatment during a critical period to improve later biological, psychological and social outcomes. In addition to community work if required it is possible to negotiate input to inpatient care for EI patients admitted to both Southmead and/or Callington Rd hospital. The trainee has the opportunity to get involved as an educational supervisor and formal teaching in the 3rd year medical student programme based at Callington Road Hospital. Facilities: Desk space and computer access in room shared by 4 senior clinicians; or if preferred in larger open space team office; additional access to desk and computer in a designated quiet work room. The trainee will work alongside Dr Linda Heaney in South Bristol Recovery Service. There are 3 Consultant Psychiatrists, Dr Linda Heaney, Dr Susanne Wille and Dr Ane Gillet in the Team. The post is based at Petherton Resource Centre in South Bristol, where the trainee will have office and computer facilities, and secretarial support. The Recovery service incorporates aspects of the CMHTs, Assertive Outreach Service, Assessment and Intervention and Community Rehabilitation teams, and aims to merge best practice of these teams. We work closely with the crisis team, who are co-located with us. The team atmosphere is friendly and supportive. South Bristol is a diverse mix of an inner city component, with areas of larger housing estates. The post holder will become skilled in dealing with the challenges of working in an area of social deprivation. There are a range of hostels and community rehabilitation homes, and the trainee will gain experience of working in these. The post is community based, and the post holder will learn the advantages and disadvantages of the functionalised model, and will interact closely with the Inpatient and Home Intervention Teams. There are ample opportunities to do new assessments within the team. A particular emphasis is placed by the whole team to support patients’ personal journeys to recover from their conditions, and to work with them to reduce the risk of future relapses. The post offers the opportunity to become skilled in the use of the Recovery Model. This can be achieved through clinical practice and supervision. There are opportunities to be involved with the use of the Mental Health Act, including CTOs and Home Office patients, assessing new referrals and supervising junior doctors. Dr Heaney also provides medical cover to the South Bristol clients with the Deaf Mental Health teams. There are ample opportunities to participate in teaching and training of medical students and junior doctors, and to participate in the audit process. Dr Heaney has a particular interest in medical education, and interest in this will be supported and encouraged. There is a weekly postgraduate educational program at the nearby Callington Road Hospital, where there are also excellent library facilities. In this new post, you will have the opportunity to work in an Inner City assessment and recovery service. Inner City Psychiatry could be regarded as a subspecialty in its own right. You will have the unique opportunity to work with the most diverse community in the Deanery, and begin to understand this population’s particular needs. It will provide a valuable opportunity to truly engage with the role of culture and context in mental disorder. There is ample opportunity to work with asylum-seekers and refugees, in many different ways. There is a golden opportunity to work closely with local GPs and understand the challenges and rewards this interface can bring. You will join the senior clinicians in the team, and will be encouraged to see yourself as a clinical leader as well as a trainee. You have a lot to offer this team, and will be encouraged to get involved in service and team development. You will also be able to undertake regular clinical work to further develop the skills they have acquired as a core trainee. This will include: completing regular initial assessments; taking on longer term individual work (if they would like to, and agree as part of their learning objectives for the post); reviewing patients at the request of care coordinators; completing emergency assessments; gaining experience producing reports for MHA managers or MoJ (if they wish); having the opportunity to attend MHAAs; attending regular assessment review meetings; attend team meetings (including reflective practice meetings); working closely with multiple other agencies including CJS, child protection, drug and alcohol services, general hospitals, third sector organisations, organisations working with homeless people and those who work with particular ethnic and cultural groups. The trainer’s main interests are in psychosis, psychotherapy and history and philosophy of psychiatry. There will be opportunities to develop psychotherapy skills if you would like to do so, and to research in the area of psychosis via Psychosis Health Integration Team (which the trainer is director of). All of this would be guided by your own interests and development needs. There will be the opportunity to pursue teaching undergraduates and take part in post-grad educational meetings locally. You will be encouraged to teach other members of the team, and the team as a whole. The team has medical students, nursing students, social work students and trainee psychologists and psychotherapists (whom the trainer supervises). You will also be able to provide support to the core psychiatric trainee in the team. You will have full access to secretarial support, including support with any letters from on-call work, and have access to a computer (both desktop and laptop) and have a desk allocated to them. You will have a lockable drawer to store work and personal items. You will receive 1 hour dedicated individual supervision with the trainer. This will take place in a quiet, private room. The timetable of trainer and trainee overlap for much of the week so there will be regular face-to-face support available at most times, and by phone at other times. You will be encouraged and supported to develop their own learning needs for the post, and will be supported to develop your special interest time to further those. You will have the opportunity to attend a local Balint Group; and encouraged and supported to use study leave. There will be no requirements to cover the service or other doctors’ work (therefore flexibility for annual leave). Any enquiries or visits from interested trainees are welcome. This post is based at North Bristol Assessment and Recovery Team at Greenway Community Centre, Doncaster Road, Southmead, Bristol. The team covers population across North Bristol and works across three GP based work streams and covered by 3 WTE consultants and 0.6 WTE Old Age Consultant. The team is multidisciplinary managed by Team manager and band 7 senior practitioner including band 6 specialist recovery worker (CPN), occupational therapist, physiotherapist, band 5 recovery navigator and band 4 recovery navigators. The team has imbedded crisis intervention team working along with our team to provide coordinated crisis team support. The team has dedicated Complex Psychological Intervention (CPI) team linked which will provide psychological services. The medical team consists of one full time and one part time specialty doctor, GPVTS trainee and Core trainee (CT 2). The recovery team offers Primary Care liaison services and performs assessment of referrals from GP and other specialists to secondary care mental health services. The role of the SpR will be to gain experience and expertise in the diagnosis, assessment, management and prevention for the full range of mental health disorders affecting adults of working age and older adults (later life). The trainee will get experience of jointly assessing, working and supervision of complex cases from Complex Psychological Intervention team. There will be ample opportunities to see patients with disorders ranging from schizophrenia, Mood disorders through to personality disorders, both in clinic and through joint home visits with Care Coordinators. There will be opportunity to gain experience in assessing patients of older adults having functional and organic illness. There will be opportunities to gain experience in a variety of settings, including both the outpatient and inpatient unit (the Inpatient experience at Oakwood ward will be clinically supervised by Dr Jacek Colsut). There will be good experience in multi-disciplinary working, with the opportunity to develop clinical leadership and management skills. This site has close links with Eating Disorder Services with inpatient unit, Mothers and Baby Unit, Burden Centre (Neuro-psychiatry unit), Oakwood in patient unit with 23 beds and 4 bedded place of safety unit at Mason ward at Southmead Hospital. There is a psychiatry liaison services with dedicated consultant at Southmead hospital which provides a unique experience of general wards and A&E. There are strong links with Psychiatry department at Bristol University and has ample opportunities to develop academic skills and expertise. The post will give the SpR an opportunity to look at Mental Health Act assessments and implementations, preparing reports for tribunals as well as looking at developing expertise in the pharmacological, physical and psychological treatment approaches. The SpR will be directly supervised by the Consultant with weekly formal supervision and on-going informal supervision on a day to day basis. The SpR will gain management experience through organising the Southmead Academic Programme as the supervising consultant is College Tutor for the site. There is plenty of scope to develop teaching skills through teaching and training Core trainees and medical students who are placed with the team. There is an opportunity to become involved with audits at Southmead Hospital, or with in North Bristol Recovery team, or with the Trust Audit Group. There will be ample opportunities to be involved in research projects. The team provides opportunities to develop management skills and the flexibility in working will support higher trainee in that role. The SpR will have opportunities to chair team meetings, assessment feedback and allocation meetings, attend Bristol MAG meetings, participate in Senior management meeting of the team, represent consultant in other Bristol management meetings. The SpR will have opportunities to attend meetings with GP as part of developing links and relationships with GP with new services. Representing consultant in Tribunal meetings and writing court reports. The higher trainee has dedicated workplace and dedicated computer, laptop and mobile phone to support in working. The SpR is supported by medical secretary who will provide administrative support for their clinical work, research, nonclinical and on-call work. The time table allows flexibility to suite working of SpR and allows formal and informal supervision by consultant. The supervising consultant is available throughout the week and has dedicated room where the supervision and discussions with SpR can take place.

Lime unit is a 23 bed acute inpatient unit. It provides safe and recovery focused environment of care for psychiatric patients mainly from the South and North of Bristol, as well as out-of-area patients. The patients’ clinical profiles are widely varied, involving all acute and major psychiatric disorders as well as dual diagnosis, borderline learning disabilities, physical co-morbidity and complex personality disorders. The acute inpatient unit is placed within the care pathway of the functional model hence working here encourages very close collaboration with all other psychiatric teams, on a regular basis through the CPA and ward reviews and care planning process. We work particularly closely with the Bristol South CRHT, which partly operates from the ward including facilitation of discharge (FD). Callington Road Hospital is a GMC approved training site for Core and Advanced training in psychiatry. It is expected that the Advanced Trainee in this post would be interested in the acute care pathway, working directly with the supervising consultant and the multidisciplinary team to offer assessment and treatment of a wide range of conditions within general adult psychiatry. Typically the majority of the inpatients are detained under the Mental Health Act, and the average length of stay is less than a month. All admissions are triaged and gate-kept by the Bristol Intensive Team with whom we have close links and communication. The inpatient team interfaces with a range of services including Bristol South and North Recovery Teams, Bristol Early Intervention Team, as well as Male and Female PICUs and Rehabilitation services. There is a 1:16 non-residential on-call rota. The post holder will have the opportunity to focus on a range of different aspects of acute care, depending on individual preference and educational requirements. These include a focus on legal issues including report writing and presentation at Mental Health Review Tribunals, Community Treatment Order meetings, CPA and professionals meetings, vulnerable adults and MAPPA meetings. There are a range of teaching opportunities, including at the weekly Callington Road Hospital academic programme, journal club, and peer supervision of junior trainees. Throughout the academic year Lime Unit hosts at least 6 medical students from Bristol University during their psychiatry placements. This post is based at Oakwood Ward, Southmead Hospital Bristol and the post holder will work with Dr Jacek Kolsut. Oakwood unit is a 23 bed acute inpatient unit. It provides safe and recovery focused environment of care for psychiatric patients mainly from the South Gloucestershire and North of Bristol, as well as out-of-area patients. The patients’ clinical profiles are widely varied, involving all acute and major psychiatric disorders as well as dual diagnosis, learning disabilities, physical co-morbidity and complex personality disorders. The medical team on the unit is also supported by CT doctor, FY doctors and a Trust Grade doctor. The post holder will have the opportunity to focus on a range of different aspects of acute care, depending on individual preference and educational requirements. This post provides exposure to community psychiatric practice with an enthusiastic trainer across a patch covering inner city and rural areas that encompass some of the most and the least deprived areas of Gloucestershire. There is the opportunity also to link in with adult inpatient work with Dr Luffingham, based at Wotton Lawn Hospital. This mix provides for a very varied caseload and the opportunity to develop skills in a variety of styles of management. The Recovery team is responsible for assessing and treating new referrals from primary care and other agencies and for the ongoing treatment of patients with complex mental health problems including severe and enduring mental illness. Our setting, resource level and approach allow us to offer advanced trainees excellent and tailored opportunities for training and professional development that would suit both those transitioning from core to specialist training and those preparing to take a consultant post. Trainees will be able to do independent and joint assessments (ie. with medics, our psychologist and other professionals) of routine and urgent referrals and, depending on their needs and experience, they will be able to build a caseload of patients care co-ordinated by other team members. Multidisciplinary working will be expected with patients in the community and it would be expected that the trainee would get opportunity to both work with patients referred to CRHTT and to do Mental Health Act Assessments. The post also has the opportunity to link up with other local teams, eg AOT, Older Adult, Eating disorders, Liaison etc. The team is based at Avon House, Tewkesbury, but also works out of Leckhampton Lodge, Cheltenham. The medical support for the team is provided Dr Christopherson and Dr Milroy, Specialty Doctor. The multidisciplinary team consists of: 1 x team manager, 2 x 0.5wte OT, 2 x 0.5wte social workers, 3 x band 6 CMHNs, 1 x band 5 CMHN, 1 x vocational worker, 1 x psychologist and 2 x support workers. GPVTS trainees and medical students are commonly present. The consultant has a strong background in psychological therapies, has mentored new consultants and has supported people to develop their personal branding and their interview skills. He chairs the MSC, convenes the Recovery Consultants’ meeting and leads on Anxiety Disorders within the trust Functional Expert Reference Group. He is the Finance Officer for the South West RCPsych Division. The trainee is encouraged to attend the local academic programme and will be facilitated to gain management experience. The post is based with the Gloucestershire Recovery in Psychosis (GRIP) team, Gloucestershire Health and Care NHS Foundation Trust, which assesses and manages individuals between the ages of 14 and 35 who are suffering first episode psychosis (FEP), and monitors persons who present with At Risk Mental State. (ARMS). The multi-disciplinary team covers the entirety of Gloucestershire (Population 600,000) and is divided into three sub-teams to cover different geographical areas. The consultant and specialty doctor provide medical input into each team; the trainee may choose to focus on one sub-team, or work across all teams. The GRIP is well resourced, to work a maximum caseload of 15 per WTE clinician. The team is motivated and supportive; it comprises 1 consultant, 1 specialty doctor, 1 team manager, 2 clinical psychologists, and a full multidisciplinary team comprising nurses, occupational therapists, social workers, support workers and sports therapist/ physiotherapist. The team base is 44 London Road , London Road , in the centre of Gloucester. Patient contacts occur in several mental health bases across the county, GP surgeries and patients’ homes. The administrative support to the medical team comprises a dedicated full-time secretary. The trainee would have a desk, computer and filing cabinet, and may chose to share an office with medics, or share an office with other team members. Clinical duties are restricted to patients over the age of 18, and include assessments, MHA assessment, medication review and prescribing, and collaborative working to a recovery model with other team members. There is opportunity of preparing medicolegal reports under supervision. Risk assessment is core business for the team as the patient group are often impulsive and disturbed. Local special interest sessions include: Wotton Lawn inpatient unit, and Montpelier Low Secure Unit (which has links with Gloucester Prison Mental Health Team). There will be weekly formal individual supervision. The timetable offered is flexible to pursue existing research, teaching projects and special interest commitments. This post is based at the Montpelier Unit (a Low Secure Inpatient Ward) on the Wotton Lawn Hospital site in Gloucester. This post provides excellent experience in General Adult Psychiatry (non-core), particularly in the interface between General Adult Psychiatry and Forensic Psychiatry. The Montpelier Unit provides a countywide service to the residents of Gloucestershire (population 580,000) in the form of a 12 bedded low secure inpatient facility for patients with severe mental illness who require treatment in a low secure setting. Patients are expected to stay no longer than two years in the Unit before moving on, usually back into the community. It is envisaged that most admissions will come from one of three groups: patients descending the security ladder from high and medium security, prisoners with acute and severe mental illness requiring inpatient hospital treatment and patients with severe mental illness who challenge the capacity of open and PICU wards locally to manage their problems appropriately. The Montpelier Unit is an all male facility. In addition to this the Unit runs the local Gloucestershire Courts Criminal Justice Service(GCLS) and offers an opportunity for training in ECT.The role of the trainee is to develop skills in the assessment and management of patients a) referred for ‘access assessments’ for admission to low security. b) referred to the GCLS. The Montpelier Unit is part of the county wide directorate. The Advanced Trainee will have the opportunity to teach Core Trainees both formally as part of the Monday afternoon training Programme and also informally. There will also be opportunities to train other disciplines from the Montpelier Unit and also opportunities for training other professions within the Criminal Justice System such as Prison Probation Staff. General Adult Psychiatry post, based at the Crisis Resolution and Home Treatment Team (CRHTT) in Gloucester and the Forest of Dean. The CRHTT is a strong multi-disciplinary team with experience in working with doctors in training. Two consultant psychiatrists work with the team and there is a staff grade doctor who supports the inpatient consultant psychiatrist. The team works collaboratively with Primary and secondary care services. The post will offer experience of acute psychiatric care in the community and an opportunity to lead the treatment for patients who are admitted to Wotton Lawn Hospital, Gloucester and were under the care of the CRHTT. The advanced trainee will have an opportunity to increase their expertise in assessment and management of risk and in home treatment of a wide range of mental health disorders with different complexity. The advanced trainee may chose offer day to day clinical advice to the staff grade psychiatrist attached to the inpatient consultant. There will be opportunity for mental health act related work – assessments, writing reports for and attending mental health review tribunals and community treatment order meetings. The CRHTT also provides home treatment to older adults and the post may suite trainees in Old Age Psychiatry training who would like to increase their experience in acute psychiatry. A protected office space with a desk, a filing cabinet and computer are designated for the specialist trainee at the CRHTT base. Secretarial support is at hand too. Protected travel allowance is available for trainees based in Bristol/Bath and the Advanced Trainee can be on-call from out of county. Dr Vrabtchev works two days as a consultant medical psychotherapist in Cheltenham and North Cotswolds area and is the Psychotherapy Tutor for Advanced trainees and staff grade doctors. There are local opportunities for special interest session(s) in psychotherapy under Dr Vrabtchev’s supervision. Other local special interest sessions include – AOT team, Early Intervention in Psychosis, low secure inpatients and prison in-reach, liaison psychiatry and nurse led eating disorders team. Dr Vrabtchev is the lead in personality disorders service development within the Trust and is involved in service development and training projects. The trainee’s interest and involvement in such projects will be encouraged and supported in order to develop skills in leadership, service development and teaching. Facilities The post would be mostly community based and in Taunton, covering a catchment area of approximately 65,000. All the Taunton community teams are located at Foundation House on the Wellsprings Hospital site where the General Adult and Old Age inpatient wards are based. Office space is in short supply but there is a large, shared office with secretarial support and a computer. Duties, Roles and Responsibilities Services are comprehensive and include Assessment, Home Treatment, Recovery, Early Intervention and Assertive Outreach teams. It is expected that the post holder would work more closely with one of these teams depending on their interest/training requirements. In addition there is a strong Psychological Therapies network and an opportunity to see psychotherapy patients under supervision. The two main psychotherapy models are CBT and CAT. Although the consultant roles are split between inpatient and outpatient settings, continuity is promoted by mapping inpatient sectors on to community sectors. Inpatient experience is available under the supervision of Dr Waqar Ahmed, the inpatient consultant, and this can be a core post if the postholder wants. There is a part time associate specialist working in the community and a psychiatric/GP trainee based on the inpatient unit, in addition to an F2 trainee. The postholder would be expected to participate in the senior Out of Hours on-call rota for telephone calls and referrals up until midnight, at which point the second doctor on the rota would take over. Currently this is a 1 in 10 rota. There would be the opportunity to observe/participate in Mental Health Act Assessments and tribunal work. There are a range of teaching opportunities, including the weekly PGME meetings in Taunton to which the postholder would be expected to attend. They will also participate in peer supervision and peer led training of junior trainees. Throughout the academic year we have at least eight medical students on attachment from Bristol University. Local teaching is organised for them and the postholder would be encouraged to participate. The trainee will be supervised by Chris Mortimore, a full time Consultant in General Adult Community Psychiatry. The post is based with the Bridgwater Community Mental Health Team at Glanville House. This base is located 45 minutes by car from Bristol and is easily accessible from the M5. The trainee will gain experience across a wide range of general adult psychiatric disorders, working with the supervising consultant and with the multidisciplinary teams in a community setting. There is one full time general adult consultant psychiatrist (Chris Mortimore) and there are also two part time consultants (Martin Eales, Anton Muller) based at Glanville House. The medical team provides senior medical advice to the Somerset Coast Crisis Resolution / Home Treatment Team, the Bridgwater Community Mental Health Team, the Somerset Coast Assertive Outreach Team and the Early Intervention in Psychosis Team (STEP). The trainee will therefore have opportunity to work closely with these services, all of which are based at Glanville House. There will be opportunity for Mental Health Act work. There is a psychotherapy service at Glanville House which includes clinical psychology, family therapy, cognitive behavioural therapy (CBT), cognitive analytic therapy (CAT) and dialectic behaviour therapy. There are opportunities for training under supervision in CBT and CAT. There is also a specialist personality disorder lead. Consultant leave is coordinated so that at least one consultant is available for urgent advice and supervision. The whole population for Dr Mortimore’s post is 45,000 and covers GP surgeries east of Bridgwater located in Highbridge, Burnham-on-Sea, Brent and the Polden Hills as well as Victoria Park Surgery in Bridgwater. The trainee will always have access to a dedicated workstation equipped with a computer, internet access, lockable filing cabinet and both a landline and mobile telephone. The consultant medical team has 1 WTE Band 4 Medical Secretary who will provide secretarial support to the advanced trainee for all aspects of their job including out of hours on call work. Dr Mortimore is a former Medical Director for Somerset Partnership and management experience can be gained through a bespoke arrangement if required. He also contributes to the ECT Clinic in Taunton and therefore clinical experience in ECT is encouraged. A wide variety of other special interest opportunities are also available. Teaching is actively encouraged. Trainees have the opportunity to be Educational Supervisor for third year medical students and are encouraged to participate in the student teaching programme delivering teaching sessions and workshops. There are also opportunities for teaching core trainees, GP trainees and Foundation doctors. Some trainees have acted as facilitators for the peer led training meetings held regularly as part of the local weekly Postgraduate Medical Education programme, which also has a mix of case presentations, speakers and journal club. Research and audit are also strongly encouraged. Somerset Partnership takes part in some national research projects and there is an active medical audit department. Dr Mortimore regularly acts as clinical lead for Trustwide audit projects and there will be opportunity for participation in audit under supervision. The trainee will join a 1:12 non-resident on call rota. This includes Section 12 work in Somerset under consultant supervision. The banding is 20%. On call duties for advanced trainees cease at midnight on both weekdays and weekends. |

This is an exciting new post in which the trainee will have the opportunity to gain experience and input working with the North Wiltshire Intensive Service (NWIS) at Green Lane Hospital in Devizes, Wiltshire. The NWIS team is a general adult community team for working age adults previously known as the Crisis Home Treatment Team. The NWIS is a 24 hours service, gate-keeping beds and facilitating early discharge in daily liaison with the Inpatient Units. This service provides: •Intensive, home based interventions as an alternative to hospital admission •Admission into/timely discharge out of acute inpatient care •Emergency face to face assessments within four hours of accepting a referral •Urgent face to face assessments within seventy two hours of accepting a referral. The team works closely with the inpatient unit to provide early discharge and home treatment for people following admission and arrange appropriate follow up as necessary. Home Treatment will be provided to “wrap-around” Care Co-ordination in Recovery Teams to prevent hospital admission or to facilitate enhanced discharge from inpatient units. The Intensive service Team attends all Mental Health Act Assessments and the NWIS Consultant is in the majority of cases part of the assessment team. This includes the local Place of Safety at Green Lane Hospital and Melksham Custody. The Higher trainee will be an integrated member of a multidisciplinary team which offers psychiatric assessment and treatment; they will work alongside the Consultant Psychiatrist. The team has also weekly case discussions with a psychologist. There is an hour’s weekly supervision session with the consultant. The consultant is actively involved in improving the acute care pathway for patients. The Higher trainee is welcome to participate in the local work on the Mental Health Crisis Concordat which addresses a wide range of aspects of the Crisis Care such as liaison with the police, ambulance services, A&E Departments etc. The consultant is appointed Medical Lead for West Wiltshire, Early Intervention and Intensive Services. The Higher trainee would be welcome to join the consultant in a variety of management meetings both locally and on Trust level. The local inpatient unit, Imber ward, is a twenty bed mixed sex unit and there are circa 5 admissions per week with a length of stay of 20 days (median). The Higher trainee would be able to negotiate experience on the ward if interested. The NWIS is based at Green Lane Hospital, Devizes which is a small and historic market town. The catchment area covered is mainly rural with other small towns and provides an eclectic mix of cases and experiences. Green Lane Hospital has a small library and a very experienced and active librarian who will willingly help with any research or information requests. The Higher trainee will have access to a workspace with a computer, bookshelves and filing cabinets. There is a secretary with the NWIS providing medical admin support. The NWIS has an open planned office with sufficient hot-desk facilities. In addition there is an office used by the Team Manager, Modern Matron and Consultant Psychiatrist with lockable drawers.
The Higher trainee is welcome to participate in audit and research according to their own interests, the Trust is particularly interested in ways to improve care and quality in today’s economic environment.
It is possible to gain teaching experience with the undergraduate and postgraduate Training. The Consultant is regularly involved in the teaching of medical students and the Higher trainee has the opportunity to take on a 4rd year medical student and get involved in the medical student teaching programme based at Green Lane Hospital. The Higher Trainee would be welcome to get actively involved in the Academic Programme/Journal Club at Green Lane Hospital. As part of their CPD the trainee is invited to attend and be actively involved in the weekly Postgraduate Meetings at Green Lane Hospital.
Experience on the Inpatient Ward can be negotiated. In addition there will be the opportunity to get involved in undergraduate and postgraduate teaching however the teaching time table has not been confirmed yet. |

This post offers Advanced Trainees the opportunity to gain extensive experience of General Adult Psychiatry in prisons (working with both male and female service users) with an enthusiastic trainer. This is very much a General Adult post, not a Forensic Psychiatry post. There is a current CT2 trainee undertaking their 6 month post with our Service and a number of trainees, both Advanced and Core Trainees have given excellent feedback regarding their experience during Special Interest sessions, which are also available. The Service underwent successful recommissioning in April 2016, which saw AWP join partner organisations, including Bristol Community Health and Hanham Health, in providing integrated healthcare across 5 prisons under ‘InspireBetterHealth’. The Advanced Trainee will join multi-disciplinary teams, in 2 local prisons, who are employed by Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) and their Consultant Psychiatrist trainer is also employed by AWP. There is a second Consultant Psychiatrist who works with our Service who could offer additional experience in 3 other prisons if the Advanced Trainee was interested to gain broader experience. The MDTs include RMNs, Clinical Psychologists, Assistant Psychologists, Social Workers, an Occupational Therapist and Learning Disability Practitioners. As secondary mental health teams we are integrated with Primary Care, GPs in the prisons and Substance Misuse Teams, which would be an interesting model for the Advanced Trainee work within, giving exciting opportunities for multi-disciplinary working and teaching. There are also opportunities to teach lay groups including prison officers. We frequently have Advanced and Core Trainees undertaking Special Interest sessions and host medical student visits to clinics, so medical student teaching is also available in this post. We are offering a Medical Student SSC this year in the prisons, which a trainee will be supervising. The Advanced Trainee will be encouraged to develop their teaching portfolio during this post with a trainer who is committed to education and teaching. The office base for this post is at Petherton Resource Centre. The trainee will have excellent secretarial support from a dedicated medical secretary, access to desk, computer and telephone at Petherton. The local Postgraduate Academic Programme is at Callington Road Hospital weekly, just 5 minutes drive away from Petherton. The Trainee timetable below is flexible, depending on the Trainee’s interests and previous experience. A comprehensive Induction to the post will be provided. This post would provide the trainee with an excellent breadth of training in developing clinical leadership in a multidisciplinary team that provides holistic ‘personal recovery’ oriented care and treatment of people in the community within a secondary mental health service. This post in based at Leckhampton Lodge in leafy Leckhampton in the regency town of Cheltenham. The trainee will have their own desk with computer and telephone in an office shared with 2 -3 other doctors and full medical secretary support. The trainee would be expected to see no more than 8 patients in the six core clinical sessions per week, as I believe in deeper holistic learning by seeing few carefully selected patients and thereby develop adequate competence and confidence in managing patients with multidisciplinary and inter-agency support which would be the fundamental role of a Consultant Psychiatrist in the first few years in that role. In the core clinical sessions, the trainee would have opportunities to: assess and formulate treatment plans for patients across a range of psychiatric diagnoses; develop supervision skills by supervising medical students and junior trainees; learn to manage interface challenges across different services, and thereby develop negotiation skills; attend weekly local academic programme and the monthly medical staff committee meeting. The trainee would have ample opportunities for developing additional knowledge and competencies in other areas by doing ‘special interest sessions’ in the Gloucestershire Early Interventions Service for People with Psychosis, the Mental Health Liaison Team, the Crisis Service, Assertive Outreach Service, Eating Disorders Treatment Service, Low Secure Unit, Medical Psychotherapy, Medical Education, Medical Management, Quality Improvement Projects etc. The trainee would be welcome to take an active role within the Fritchie Centre which is a specially developed research centre within Gloucestershire Health and Care NHS Foundation Trust that manages both NIHR funded and commercial research studies.
The Cheltenham Recovery Team is a well-functioning team managed by an excellent Team Manager and it has always welcomed and valued trainees. Four other consultant psychiatrists are based at Leckhampton Lodge should the trainee ever require clinical supervision in Dr Choudhury’s absence.

Responsibilities
The advanced trainee will be expected uphold the standards outlined in the General Medical Council’s ‘Duties of a Doctor’ at all times.
The trainee will be seeing newly referred patients for initial psychiatric assessments, formulation of diagnoses and holistic treatment plans. This would involve seeing patients in outpatient clinics and at their homes or other residential settings. All assessments would include a full psychiatric history and comprehensive assessment of all risks pertaining to the patient. They would have the opportunity to discuss the patients they see with Dr Raj Choudhury or another senior clinician in the team. They would be expected to regularly attend the weekly multi-disciplinary team meetings and also develop communication and networking skills by having clinical discussions with colleagues in the multi-disciplinary team outside the team meetings. They would be expected to build their own small caseload by following up some of the newly referred patients that they assess, and to consider discharging the patients or transferring them to Dr Raj Choudhury’s caseload or the team caseload before they complete their post. Dr Raj Choudhury will have oversight of the diagnosis and treatment plans of all patients on the trainee’s caseload.
The doctor will participate in a non-resident on-call rota on a 1:15 frequency with a Consultant Psychiatrist as second on-call. As the post is supernumerary, the trainee would not be expected to arrange prospective cover for study leave or annual leave but they would be expected to swap on-calls that fall within the leave period.
There will a weekly hour long formal supervision session with Dr Choudhury. The timetable could be flexible but the trainee would be expected to attend the weekly multidisciplinary team meeting and preferably the weekly local academic programme. The trainee will have access to a dedicated desk space in a doctors’ office for their sole use and also access to full medical secretary support for all their clinical and administrative work.

This post will be based in the North Bristol Recovery Team based at the Greenway Centre. The Recovery team covers a total population of approximately 175 000, with this being divided into three work streams (clusters), each with approximately 1.0 WTE consultant. There are 5 consultant psychiatrists working in the ageless recovery service (Drs Hodgson, Archdall, O’Shea, Rees and Agrawal). The North Crisis team spoke is also based at Greenway with opportunities to be involved in home treatment if desired. North Bristol has a diverse urban population with wide variation in ethnicity and morbidity (reflected in mental health indices). Outpatients and specialist recovery services are based at Greenway centre but patients are seen in range of settings including GP practices and Gloucester House, Southmead Hospital. In-patient services are based at Southmead Hospital and Callington Road Hospital. AWP is the lead organisation in Bristol Mental Health (http://bristolmentalhealth.org) which is a partnership of Public and voluntary sector organisations to provide NHS funded services in the city since October 2014. The North Bristol Recovery team is a multi-disciplinary team, including Specialist Recovery Practitioners (CPNs, OTs, and SWs) and Recovery Navigators. There are 3 clusters (or work streams) with dedicated GP practices- each is supported by junior trainees (CT2, GP and F2 trainee currently) and one SAS doctor supports the team. The role of the ST4-6 will be to gain experience in the diagnosis, assessment, and treatment of the full range of mental health disorders affecting adults of working age. The team is an amalgamation of past CMHTS, AOT, Community Rehabilitation and Later Life functional illness. There are opportunities to gain experiences in a variety of settings, including inpatient unit, community base, general hospital and primary care. The Assessment service is integrated within the Recovery team so a key role for the trainee is to become expert in assessing referrals from primary care. The service model promotes an increasing model of Primary Care liaison with interface meetings with GP practices with opportunities to evolve primary care liaison services and get experience in the commissioning and tendering process. The post offers experience in multi-disciplinary working, with the opportunity to develop clinical leadership skills, use of Mental Health Act (management of restricted and CTO patients), and expertise in pharmacological and psychological approaches. The Complex Psychological Interventions service (embedded) offer a range of interventions and trainees can gain experience and supervision accordingly. The trainee will be directly supervised by Dr Rees, with weekly formal supervision. The post holder will play a role (or help co-ordinate) he Academic Programme, with opportunities of trainees and medical students at Southmead Hospital. Involvement with audit, quality improvement and research projects is also expected. The team has Consultants (including Dr Rees) with medical management roles which the trainee can become directly involved with if desired. The trainee will be expected to participate in local management meetings. There is a dedicated office for Junior Medics at the Greenway Centre with access to computer/IT, dedicated desk and lockable filing cabinet available. The secretarial staff includes Dr Rees’s secretary based on-site. Administrative and secretarial support is available for all aspects of the trainee’s job, including daytime clinical work but also work generated on call and any other secretarial work generated in the execution of their duties as a psychiatric trainee. We are also fortunate to have well established links with Early Intervention, Rehabilitation, Perinatal, Dementia pathway, Liaison and Eating Disorders Service at Southmead Hospital, which will be available as special interests if required. There are also strong links with the inpatient services and the Bristol Intensive Team. There are links with Bristol University and the Student Health Service and so higher trainees are able to undertake consultant-level assessments on students in difficulty for the University (under supervision of Dr Dan Hodgson). To gain experience in becoming an Educational Supervisor, there are opportunities to support and supervise a core trainee working in the team, offer support with training and induction, and participate in exam preparation sessions undertaken by the Consultants. The NorthAcademy for undergraduates from BristolUniversity is based at SouthmeadHospital. Trainees are able to participate in formal and informal teaching, mentoring of students and become an examiner, if this is of interest. The trainee takes part in the pan- Bristol, South Gloucestershire and North Somerset senior on call rota, which covers Police Stations, Place of Safety Section 136 Suite at Southmead Hospital and multiple in-patient sites. This post, covering the Gloucester catchment area of Gloucestershire, offers experience in Core General Adult Psychiatry in acute inpatient settings. All the admissions to Wotton Lawn Hospital are gate-kept by the CRHTT, which has close links with other community teams, including the Recovery, Assertive Outreach, Early Intervention and Mental Health Intermediate Care teams. Clinical work will include review of new patients admitted to Priory ward. It would also include review of patients that are receiving treatment in hospital on an informal basis and also patients detained under the mental health act. The trainee will be able to work closely with colleagues from the multidisciplinary team on the ward and will work closely with the crisis team, Recovery services, Assertive Outreach team, Early intervention team and Mental health Intermediate care team to facilitate early discharge from the ward. There will be opportunity to focus on a range of different aspects of acute care including Mental Health Act Assessments, report writing and presentation at First Tier Tribunals and Manager’s Hearings, CPA and professionals meetings. The post has a Foundation Year 1 trainee and a core trainee. Medical students are also attached regularly and there are ample opportunities to develop teaching skills. Participation in audit and research, and study leave is supported and encouraged. The trainer is Inpatient Medical Lead for Wotton Lawn Hospital and the trainee will have opportunities to attend relevant management meetings.
